Herausforderungen und Lösungsansätze in der Notfallsanitäter-Ausbildung

Die Begriffe der Leistung und Anforderung im Kontext einer praktischen Prüfung

Damit eine Leistung prozessorientiert beurteilt werden kann, muss zunächst festgelegt werden, was überhaupt beurteilt werden soll. Im Wesentlichen geht es dabei um die Leistungen der Schüler*innen, ausgedrückt in Handlungen und deren Zusammenhängen. Hierzu ist es notwendig, sich mit den Begriffen der Leistung und Anforderungen auseinanderzusetzen, da sich diese final in den Notendefinitionen wiederfinden.

Leistung und Anforderungen innerhalb von Prüfungsaufgaben sind ein elementarer Bestandteil der Beurteilung und Bewertung, sie werden explizit in den Gesetzen (in Bayern z. B. Artikel 52, BayEUG und §8 NotSan-APrV (2013)) hinsichtlich der Notengebung verwendet werden. Daraus abgeleitet, müssen diese Begriffe innerhalb der Beurteilung und Bewertung und damit auch durch das Werkzeug berücksichtigt werden.

Im Rahmen der NotSan-Ausbildung findet für die staatliche Prüfung folgende Notendefinitionen nach §8 NotSan-APrV (2013) Anwendung:

Davon abweichend wenden die Schulen während der Ausbildung (nicht in der staatlichen Prüfung) andere Notendefinitionen an, da die Benotung selbst Ländersache ist. Ein Beispiel hierfür ist Bayern, es gilt hier das „Bayerisches Gesetz über das Erziehungs- und Unterrichtswesen (BayEUG)“ vom 31.05.2000, worin in Artikel 52 leicht anderslautende Definitionen in den Notenstufen 5 und 6 vorherrschen. Nachfolgend werden die Begriffe Leistung und Anforderungen näher beleuchtet:

Der Leistungsbegriff

Der Begriff der Leistung kann aus mehreren Richtungen betrachtet werden, zum Beispiel aus dem Blickwinkel der Physik. Eine hier zur Geltung kommende Formel beschreibt die Leistung als verrichtete Arbeit pro Zeit. So kann über sie aufgewendete Energie die Leistung sehr genau bestimmt werden und ihre zugehörige SI – Einheit sind Watt. Diese Definition erscheint schon deshalb ungeeignet, da schulische Leistungen nicht in Watt, sondern in Notenstufen ausgedrückt werden. Zielführend im Sinne der Leistungsbeurteilung und -bewertung innerhalb dieses Artikels wirkt die Betrachtungsweise von Wolfgang Klafki. Leistung im pädagogischen Sinne versteht Klafki „[…] als Ergebnis und Vollzug einer Tätigkeit, die mit Anstrengung und gegebenenfalls Selbstüberwindung verbunden ist und für die Gütemaßstäbe anerkannt werden […]“. Hellert ergänzt diese Betrachtung:Sie kann verstanden werden, […] als ein von der Schule gefordertes und vom Schüler zu erbringendes Ergebnis seiner Lerntätigkeit“. Als Maßstab für die Güte kann der Erreichungsgrad der Anforderungen herangezogen werden, weswegen die Anforderungen anknüpfend näher beleuchtet werden.

Der Anforderungsbegriff

Die Anforderungen beschreiben, in welchem Umfang die Aufgabe zu bewältigen ist. Der Begriff der „Anforderung“ muss im Kontext seiner Anwendung gesehen werden, da es verschiedene Definitionsmöglichkeiten gibt. Diese hängen auch maßgeblich von der Betrachtungsweise ab, bzw. an was oder wen die Anforderung gestellt wird. Daher wird in diesem Zusammenhang innerhalb dieser Arbeit von „Leistungsanforderungen“ gesprochen, wenn es um die Anforderungen an die Leistung der Schüler*innen geht. Der Begriff der „Leistungsanforderung“ ist jedoch noch nicht differenziert genug, um innerhalb eines Werkzeugs Verwendung zu finden. Der Autor dieses Artikels hat innerhalb der Literatur keine spezifische Definition für den Begriff der „Anforderung“ gefunden. Aus diesem Grunde wird anknüpfend der Versuch einer eigenen, zielführenden Definition der Leistungsanforderung im Kontext der Beurteilung und Bewertung von praktischen Fallbeispielen und zur späteren Verwendung innerhalb des Werkzeugs unternommen. Diese stützt sich auf Beschreibungen aus der Psychologie, dem Qualitätsmanagement und dem Requirements Engineering.

Es können je nach Aufgabenstellung und Intention der Lehrkräfte verschiedene Typen von Leistungsanforderungen innerhalb der Bewältigung von Aufgaben auftreten. Beispiele aus der Psychologie sind „[…] Eigenschaftsanforderungen (z. B. Fähigkeiten und Interessen), Verhaltensanforderungen (z. B. Fertigkeiten und Gewohnheiten), Qualifikationsanforderungen (z. B. Kenntnisse und Fertigkeiten) sowie Ergebnisanforderungen (z. B. Problemlösungen und Qualitätsstandards)“ . Da es sich bei der praktischen Prüfung um eine Dienstleistung an einem virtuellen Patienten handelt und damit auch die Qualität eine Rolle spielt, bietet sich eine weitere Definitionsmöglichkeit an. Nach DIN EN ISO 9000:15-11 ist eine Anforderung ein(e) „Erfordernis oder Erwartung, das oder die festgelegt, üblicherweise vorausgesetzt oder verpflichtend ist. […] ‘Üblicherweise vorausgesetzt‘ bedeutet, dass es für die Organisation und interessierte Parteien üblich oder allgemeine Praxis ist, dass das entsprechende Erfordernis oder die entsprechende Erwartung vorausgesetzt ist“. Dies bedeutet, dass es z. B. üblich ist, dem Patienten nicht zu schaden, dies in Anforderungen jedoch nicht explizit formuliert werden muss.

Anforderungsformulierungen mit Hilfe von Modalverben

Leistungsanforderungen erlauben innerhalb ihrer Konzeption eine zusätzliche Graduierung und Kategorisierung, was Vorteile hinsichtlich der Formulierung ihrer Erfüllungsnotwendigkeit im Fallbeispiel bietet und nachfolgend expliziert wird. Nach Grande können Anforderungen z. B. aus den Bereichen der Gesetze, Prozesse, Stakeholder oder eines Produkts entspringen und werden entsprechend ihrer Priorität mithilfe der Worte „muss“, „soll“, „sollte“, „wird“, „werden“, „darf“, „dürfen“ formuliert. Einen ähnlichen Weg der Graduierung nutzen medizinische Fachgesellschaften innerhalb der Publikation von Leitlinien, um den dortigen Empfehlungen den entsprechenden Stellenwert zu verleihen. Die Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften (AWMF) e. V. verwendet unter anderem drei Empfehlungsgrade:

Dieses Graduierungsschema kann entsprechend den Ausführungen von Grande mit dem Modalverb „muss“ erweitert werden, sodass daraus für die Formulierung von Leistungsanforderungen folgendes Graduierungsschema resultiert, erweitert um zwingend erforderliche Leistungsanforderungen. Nachkommende Tabelle ein erweitertes, vierstufiges Schema zur Graduierung von Leistungsanforderungen in einem praktischen Fallbeispiel:

Die Empfehlungsgrade aus Tabelle 2 werden für den Bereich der NotSan- Anforderungen wie folgt anhand eines Beispiels beschrieben:

Empfehlungsgrad M: Eine zwingende Anforderung lässt keinen Spielraum für die Prüfer*innen hinsichtlich alternativer Maßnahmen. Sie ist prinzipiell keine Empfehlung mehr, sondern eine Vorschrift und Bedarf bei Missachtung starker Begründung.

Beispiel: Der Eigenschutz muss zu jeder Zeit beachtet werden. Oder: Der einwilligungsfähige Patient muss vor der Medikamentengabe entsprechend einwilligen.

Empfehlungsgrad A: Eine starke Anforderung lässt dem Prüfling einen geringen Handlungsspielraum. Eine Abweichung muss im Nachgespräch gut begründet werden.

Beispiel: Es soll initial eine elektrische Kardioversion durchgeführt werden.

Empfehlungsgrad B: Eine reine Anforderung drängt den Prüfling zu einer Maßnahme. Wird diese anders oder nicht durchgeführt, ist die Konsequenz für den Versorgungsablauf nicht per se schädlich.

Beispiel: Es sollte eine Closed-Loop- Kommunikation durchgeführt werden.

Empfehlungsgrad 0: Eine offene Anforderung lässt dem Prüfling einen großen Handlungsspielraum, dem Prüfer wird signalisiert, dass sich der Prüfling zwischen Maßnahmen begründet entscheiden kann. In folgendem Beispiel ist eine Analgesie nicht per se falsch oder richtig:

Beispiel: Es kann eine Analgesie bei dem Patienten nach erfolgter elektrischer Kardioversion erwogen werden.

Für die Erstellung eines Fallbeispiels zu beachten ist, dass das Fallbeispiel als Arbeitsaufgabe und in seinem Ablauf so gestaltet sein muss, dass sich die Leistungsanforderungen darin sinnvoll wiederfinden. Folgendes Negativbeispiel mit medizinisch nicht indizierter Leistungsanforderung soll dies verdeutlichen:

EKG- Bild und tastbarer Puls: normofrequenter Sinusrhythmus;

Leistungsanforderung: „Die Prüflinge sollen eine elektrische Kardioversion durchführen“.

Kategorisierung von Anforderungen mit Hilfe des Kano-Modells

Anforderungen können mithilfe des Kano Modells kategorisiert werden. Hier werden sogenannte Basis-, Leistungs- und Begeisterungsfaktoren genannt. Erstere sind grundlegende Erwartungen. Sie fallen nur auf, wenn sie nicht vorhanden sind. Zweitere sind das, was konkret vorhanden sein soll. Fehlen diese, sorgt dies für Ärgernis. Und Dritte sind das, was zwar nicht verlangt wurde, jedoch vorhanden ist und zu Begeisterung führt . Übertragen auf eine Prüfungssituation im Rahmen der NotSan-Ausbildung könnten folgende drei Leistungsanforderungen konstruiert werden, welche die Faktoren aus dem Kano-Modell berücksichtigen durch den Autor dieses Artikels als Basis-Leistungsanforderung, Leistungsanforderung und Top-Leistungsanforderung benannt werden. Die Basisleistungsanforderungen gehören dabei zu den Anforderungen, die gegebenenfalls nicht explizit genannt, sondern vorausgesetzt werden.

Basis-Leistungsanforderung:

  • Der Eigenschutz muss zu jeder Zeit beachtet werden.
  • Der Patient sollte entsprechend seiner Kreislauffunktion gelagert werden.

Leistungsanforderung:

  • Der Patient soll initial elektrisch kardiovertiert werden.

Top-Leistungsanforderung:

  • Es sollte eine Closed-Loop- Kommunikation durchgeführt werden.
  • Die nächsten Schritte sollten vorausgeplant sein.
  • Die nächsten Schritte sollten entsprechend kommuniziert sein.

Es darf immer nur eine Maßnahme in der Leistungsanforderung angegeben werden, da ansonsten die Bewertung bei einer nur teilweisen Erfüllung erschwert ist.

Was hier unter „Situationsangemessen“ zu verstehen ist, hängt von der Beschreibungstiefe der Anforderung ab. Je detaillierter die Beschreibung der Leistungsanforderungen, desto objektiver wird die Prüfung, da der Interpretationsspielraum der Prüfer*innen eingeschränkt wird. Allerdings werden die Prüflinge in ihrer möglichen Performanz immer weiter eingeschränkt. Dies bedeutet, dass es einen situativen Ermessensspielraum innerhalb der Beurteilung geben muss. Eine Arbeitsdefinition für eine Leistungsanforderung kann daher folgendermaßen lauten:

Eine Leistungsanforderung im Kontext einer praktischen Prüfung von NotSan ist eine Erwartung oder ein Erfordernis, welches explizit oder implizit an die Prüflinge gestellt und in deren Performanz und Reflexionsgesprächen sichtbar wird. Eine Leistungsanforderung kann sich sowohl auf die Prüflinge selbst, deren Handlungsprozess(e) oder -produkt(e) beziehen. Anforderungen können mithilfe von Modalverben hinsichtlich ihrer Priorität beschrieben und mithilfe des Kano-Modells kategorisiert werden.
